Most recent results and all current medications & supplements. If you haven’t had labs in last year, please contact me. YOU DO NOT NEED HORMONE TESTING

• Instructions: sonora quest preferred, go fasting (water only after midnight), no supplements with biotin for 3d prior, DO NOT TAKE OR USE ANY MEDS/SUPPLEMENT BEFORE TEST

• If needed, schedule appointment on-line to review labs

• I review results usually once a week & will contact you w/ feedback

New Patients: $200 TELEMED ~45 minutes

Return Patients $100 ALL VIA PHONE <30 minutes

Contraception Prescriptions: $50

For any patient, new or established, 13+ years of age, telemedicine for prescription for contraception or emergency contraception
