Payment is expected at time of service.

“Out of network” services” can submit payment receipt to insurance for reimbursement/utilize HSA funds if you choose.

Reimbursement cannot be guaranteed & deductibles may apply; it is recommended you contact your insurance company for more info.

Labs, imaging, and medications are covered based on the benefits of your specific insurance plan and any deductible requirements.

Sliding scale may be an option for those in need.

New Patients: $200 (Military discount 25% off);

Return Patients $100; via phone unless otherwise arranged

Contraception Prescriptions - $50

For any patient, new or established, 13+ years of age, telemedicine for prescription for contraception or emergency contraception.

Late cancellation (<24 hours) or no-show: $75
