Fees and Insurance Information

• NEW APPOINTMENTS: 45m via telemed $250 (Military 25% off)

• FOLLOW-UPS: <30m via phone: $100 (required annually)

• Contraception Prescriptions: $50

  • For any patient, new or established, 13+ years of age, telemedicine for prescription for contraception or emergency contraception

• Payment is expected on day of service.

• Cancellations require 24 hour notice: Late cancellation or no-show: $75

• ALL INSURANCE IS OUT-OF-NETWORK: Reimbursement cannot be guaranteed & deductibles may apply, including laboratory testing & imaging; contact insurance company for more information. HSA eligible.

  • You do NOT need labs to get started. YOU DO NOT NEED HORMONE TESTING

• Instructions: Sonora Quest preferred, go fasting (water only x 8-12h), no supplements with biotin for 3d prior, DO NOT TAKE OR USE ANY MEDS/SUPPLEMENT BEFORE TEST

  • If needed, schedule an appointment to review labs

  • I review results usually once a week & will contact you w/ feedback, if needed
