• Instructions: sonora quest preferred, go fasting (water only x 8-12h), no supplements with biotin for 3d prior, DO NOT TAKE OR USE ANY MEDS/SUPPLEMENT BEFORE TEST

  • If needed, schedule appointment to review labs

  • I review results usually once a week & will contact you w/ feedback

Fees and Insurance Information

NEW APPOINTMENTS: 45M VIA TELEMED AZ TIME $250 (Military 25% off)

FOLLOW-UPS: <30M VIA PHONE AZ TIME $100 (required at least annually)

Contraception Prescriptions: $50

For any patient, new or established, 13+ years of age, telemedicine for prescription for contraception or emergency contraception

Payment is expected on day of service.

ALL INSURANCE IS OUT-OF-NETWORK: Reimbursement cannot be guaranteed & deductibles may apply, including laboratory testing & imaging; contact insurance company for more information. HSA eligible. Receipt can be submitted to insurance for reimbursement.
