1. I am not taking NEW patients at this time, except for those with history of:

   • breast cancer, post-surgery for genetic mutations, abnormal mammogram,

   • clotting disorders or DVT,

   • premature ovarian insufficiency, early menopause,

   • endometriosis,

   • inadequate symptom control or side effects despite MHT adjustments,

   • direct referrals from other providers

   • other conditions/diseases requiring assessment of how MHT may impact said condition

2. Do you live in AZ? Yes, proceed with enrollment. No, I cannot prescribe for patients outside of AZ

3. Practice currently limited to perimenopause/menopause, if unsure if this applies, contact before step 1

Fees and Insurance Information

• NEW APPOINTMENTS: via telemed $300 (starting 8/1/2026

• FOLLOW-UPS: <30m via phone: $100 (required annually); if 15m+: $150 (starting 8/1/2026)

• Contraception Prescriptions: $50

  • For any patient, new or established, 13+ years of age, telemedicine for prescription for contraception or emergency contraception

• Messages requiring response >5m: $25

• Payment is expected on day of service.

• Cancellations require 24 hour notice: Late cancellation or no-show: $75

• ALL INSURANCE IS OUT-OF-NETWORK: Reimbursement cannot be guaranteed & deductibles may apply, including laboratory testing & imaging; contact insurance company for more information. HSA eligible.

• Instructions: Sonora Quest preferred, go fasting (water only x 8-12h), no supplements with biotin for 3d prior, DO NOT TAKE OR USE ANY MEDS/SUPPLEMENT BEFORE TEST

  • If needed, schedule an appointment to review labs

  • I review results usually once a week & will contact you w/ feedback, if needed
