Fees and Insurance Information
New patient visit (telemedicine), $300. Once treatment plan is stable, ongoing prescriptions typically transition back to PCP.
Follow-up, <30 min, phone: $100 (required annually)
Follow-up, >30 min, phone: $150
Contraception prescription: $50
Messages requiring more than 5 min response: $25
Late cancellation/no show: $75 (24h notice required)
ALL INSURANCE IS OUT-OF-NETWORK: Reimbursement cannot be guaranteed & deductibles may apply, including laboratory testing & imaging; contact insurance company for more information. HSA eligible.
New Patient Guidelines
My practice is currently limited to complex perimenopause/menopause: New patients ONLY if you have one of the following:
Post-hysterectomy
Breast cancer history — including post-breast cancer, genetic mutations (e.g., BRCA),
A clotting disorder or history of DVT
Premature ovarian insufficiency or early menopause
Endometriosis
Ongoing symptoms or side effects not resolved by prior MHT adjustments (pellets, compounded, etc)
A direct referral from another provider
A complex medical history requiring specialized assessment of how MHT may affect an existing condition
